What I can assess

Assessments in the comfort of your own home

An assessment helps you, your family and your care team understand what is happening and what support could help. Doing it at home means I see your real surroundings and daily routine.

Health assessments

A whole-person check of how you are going: vital signs, mobility, skin, continence, sleep, mood and daily living, with a clear summary of what I found.

Post hospital reviews

A visit soon after you come home to check you are recovering safely, understand your discharge plan, and have the support you need.

Medication assessments

A nursing review of your medicines: what you take, how you take them, side effects and any concerns to raise with your GP or pharmacist.

Cognitive assessments

Screening for changes in memory and thinking, using recognised tools. This is not a diagnosis, and results are shared with your GP for follow-up.

Nutrition assessments

A check of your eating, weight and hydration, and signs of malnutrition, with practical suggestions and referral to a dietitian if needed.

Pain assessments

Listening to where and how you hurt, how pain affects your day, and passing clear information to your doctor so treatment can be reviewed.

Supporting documentation for My Aged Care

Written clinical information to support a request for a My Aged Care reassessment when your needs have changed.

A clear written summary

Findings and recommendations in plain language, shared with your GP, provider or family, with your consent.

Good to know

What assessments are, and aren't

Nursing assessments

My assessments are carried out by a registered nurse and describe what I observe and find. Cognitive and nutrition assessments are screening only, not a diagnosis. Your GP or a specialist makes diagnoses and decides on treatment.

My Aged Care

I do not carry out the official My Aged Care assessment. If your needs have changed, I can provide supporting clinical documentation to help you or your provider request a reassessment.

Urgent concerns

An assessment is not for emergencies

Call 000 if:

  • There is chest pain, severe breathing difficulty or a suspected stroke
  • There is sudden confusion, collapse or severe pain
  • You believe someone is in immediate danger

For other urgent health concerns, contact your GP or an urgent care service the same day.

Receiving Support at Home?

Under Support at Home, a registered provider arranges your services, and participants do not contribute to clinical support costs such as nursing. A good first step is to ask your provider whether they can arrange nursing assessments from me. You are also welcome to contact me directly to talk through your options.

Where I visit

Central Coast: Gosford, Erina, Terrigal, Wyong, Chain Valley Bay and surrounding suburbs.

Also visiting: Newcastle, Hornsby and surrounding areas.

Not sure whether I visit your suburb? Get in touch and I will let you know.

Questions

Assessment FAQs

Do I need a referral?

You do not need a GP referral to get in touch. With your consent, I may share findings with your GP or provider.

How long does an assessment take?

It depends on what is being assessed. I will give you an idea when you call, and we will not rush.

Can a family member be there?

Yes. Family members are welcome to take part, and you decide how much involvement they have.

Can a provider refer a client to you?

Yes. Providers are welcome to get in touch to discuss referrals for assessments and nursing care.

Do you visit people in aged care facilities?

No. I only visit people in their own homes.
